Ride-Share Program Enrollment Form

The SOT Annual Meeting and ToxExpo Ride-Share Program is available to each meeting registrant who voluntarily enrolls in the program. By participating, each individual is agreeing to and accepting the following terms:

1. SOT does not (a) screen participants, assess the ability of the driver, or the state of repair of the driver’s vehicle; (b) make any determination as to the appropriateness of any resulting ride share; or (c) represent that any ride share will prove to be satisfactory to the participants.
2. Each individual participant is responsible for communicating with and working out an agreement with a potential ride sharer. SOT only maintains the list of potential ride sharers but does not make any ride-share arrangements.
3. After ride matching, the registrants are required to remove their names from the ride-share list.
4. Agreeing to the waiver of liability (included in the submission form) is a condition of participating in the Ride-Share Program.

*   I acknowledge that the Society of Toxicology’s role in the Ride-Share Program is limited to maintaining the list of potential ride sharers and that SOT does not (a) screen participants, assess the ability of the driver, or the state of repair of the driver’s vehicle; (b) make any determination as to the appropriateness of any resulting ride share; or (c) represent that any ride share will prove to be satisfactory to the participants. I hereby accept full responsibility for the addition of my name to the list of meeting registrants interested in ride sharing and full responsibility for the outcome, predictable or otherwise, of (a) my use of the service and (b) any resulting arrangement which I may make for the sharing of transportation to the Annual Meeting. I hereby release SOT, its officers, employs, agents, and assigns from any and all liability for claims, damage, and/or injury resulting out of my use of this service. Required
